What causes a false positive urine drug test?

I went to the ER one night because I was having chest pain. They took a urine test on me and tested for drugs. The doc came in and told me it tested positive for LSD and cocaine. I have never done those in my life. I was arguing with the doctor for about 25 minutes that I have never done any drugs. So what could cause this? The only medication I'm on is Synthroid for hypothyroidism and Toprol XL for Atrial Fibrillation.
